Report an Incident
Use this form to report experiences of racial discrimination, bias, or microaggressions on campus. Your report will be reviewed by designated Black@ASU administrators and may then be forwarded to the Dean of Students office or other appropriate staff for follow-up.
Privacy Notice: You may submit this form completely anonymously. We will not log your IP address or email unless you explicitly provide it for follow-up. Please note that an anonymous report may limit the university's ability to formally investigate.
